Semaglutide
Semaglutide is a GLP-1 receptor agonist developed by Novo Nordisk. It is FDA-approved as Ozempic for type-2 diabetes, Wegovy for weight management, and Rybelsus as an oral formulation.
Mechanism
- GLP-1 receptor agonism. Slows gastric emptying, reduces appetite via central nervous system pathways, enhances glucose-dependent insulin secretion.
Common research protocols

- FDA-approved weight management dosing. Weekly subcutaneous injection, titrated from 0.25 mg through 2.4 mg over 16-20 weeks.
- Titration. Standard escalation: 0.25 mg → 0.5 mg → 1 mg → 1.7 mg → 2.4 mg at 4-week intervals.
Evidence at a glance
- Human data. STEP-1 trial (Wilding et al., 2021, NEJM): mean weight reduction of approximately 14.9% at 2.4 mg over 68 weeks.
- Cardiovascular outcomes. SELECT trial (2023) reported reduced major adverse cardiovascular events in overweight/obese adults with established cardiovascular disease.
- Known side effects. Nausea, vomiting, diarrhea, constipation. Rare: pancreatitis, gallbladder events.
